What is the title of your proposed class?

When would you like to teach your class? Please give 3 options. You may indicate specific dates (i.e. October 15th) or general (Wednesdays in November). You must include a proposed month.

Where would you like to teach your class? You may indicate your home address, the name of a local park, or another location.

How many class sessions would you like to offer?

What age ranges are you expecting to include? Please select all that apply. 

Will there be a cost associated with your class? (Will your students need to purchase specialty supplies for your class? If so, how much do you expect it will cost per student?)

Subsidies may be available. 

Please include any other information that the board may need to make a decision about your class proposal.
